The Half Hollow Hills Welfare Trust Fund was established in September 1979 to provide union benefits for members of the district. The Trust Fund is governed by our Trustees and one Administrator. It is separate from the Teachers’ Association, has its own tax I.D. number, organization, budget and bank accounts. Currently the Trust Fund administers the following benefits; Dental, Long Term Disability, Excess Medical and Life Insurance Plans.

Beginning with this issue of the Communicator, we will discuss some of the Welfare Trust Benefits that are available to the membership:

A. Dental Benefits
1. As of January 1, 2006, reasonable and customary costs for preventative and diagnostic services (ie: checkups, ex-rays, cleanings) will be reimbursed at 100% after your deductible has been met.
2. Beginning on January 1, 2007, the maximum benefit will be increased from $1,250 to $1,500. per calendar year.

1. Excess Major Medical Benefits
• Coinsurance Reimbursement when covered expenses reach or exceed $1,625. The maximum coinsurance benefit is $1,325.
• If you exhaust $1,000,000. of Empire Plan’s Major Medical benefit, in a calendar year, the Excess Major Medical Plan will cover up to $1,000,000 of additional expenses.

C. Vision Care Benefit
Part of the Excess Medical Plan – May be used once every two calendar years (ie: use the benefit in December of 2006 – next benefit available after January 1, 2008.) If you use Davis Vision, Inc., no forms are necessary to file a claim because they administer our plan. If you use another vision provider you must obtain a vision care form.**
Examples of this benefit are:

1. Complete pair of eyeglasses (including eye exam) with frame and single vision lenses. $90.00.

2. Complete pair of eyeglasses (including eye exam) with bifocal lenses and frame - $110.00.

3. Contact lenses, including examination and fitting $120.00.
