As a benefit of the HHH Welfare Trust Fund Group Legal Service Plan, you are entitled to draft or update a Simple Will each year. The same holds true for your spouse or domestic partner. This is a service that is provided at no cost to Half Hollow Hills TA and Paraprofessional members. This year we acquired the additional benefit of being able to have a Health Care Proxy, Living Will and Power of Attorney also at no cost.
On 1/27/2010 and 1/28/2010, plan attorneys will be at the HHHTA Union office at 6268 Jericho Turnpike, Commack, NY 11725, Unit 10, from 2:00-6:00 pm to meet with legal plan participants on an individual basis to assist in drafting a Simple Will. Members, spouses, family members, or domestic partners are invited to attend the session. If you plan on attending, you will need a completed Confidential Will Questionnaire, which will be sent to you with your appointment confirmation. If you would like to take advantage of this service, please complete the coupon below and return it to Cathy at the TA office via interoffice mail as soon as possible.
