Dental Enhancements and Benefits Review for 2010 |

Beginning on January 1, 2010, the trustees of the HHHTA Welfare Trust Fund have voted to enhance our Dental Plan without increasing premiums. We are able to do this because of the good dental insurance experience we achieved in 2009/ The Maximum for General Dentistry will increase from $1750 to $1850. The Maximum for Orthodontics and Periodontal Services will increase from $500 to $600.
The following is a summary of our Dental provisions and benefits:
Coverage Period: Calendar Year (January 1 - December 31)
MAXIMUMS:
a) For General Dentistry Coverage per person per year $1850
b) For Orthodontic and Periodontal Services per year (In addition to your Max General coverage) $600
DEDUCTIBLES:
a) Individual Coverage - $50
b) Employee + Single Dependent Coverage - $100
c) Employee with Multiple Dependents Coverage
REIMBURSEMENTS:
a) For preventive and diagnostic services (after your deductible has been met) - %100
b) For all other covered services (aftre your deductible has been met) - %75
DEPENDENT CHILDREN: To age 19 or a full time student under age 23
DISABLED DEPENDENT COVERAGE: May be continued after reaching maximum age for coverage. Contact HHHTA within 30 days of the date your child's coverage ends for details and forms (499-4240).
REDUCED COVERAGE: If you and/or your eligible dependent(s) other than newborns, do not enroll in the Dental Plan within 30 days after the day you first become eligible, the amount of your coverage will be reduced for 12 months.
a) PREVENTIVE AND DIAGNOSTIC SERVICES: 100% (after a 30 day waiting period)
b) All other services - 37.5% (after a 30 day waiting period)
EXTENSION OF COVERAGE: If your employment in the district ends on June 30th, your Dental Coverage will continue through August 31st.
