One elective trust fund benefit that most union members have is our group Excess Major Medical coverage. A member or spouse must have the Empire Plan in order to participate in this plan. It is an annual policy and participating union members pay the premiums over five successive payrolls starting in January. For 2008, the yearly individual and family rates have been lowered from $120 to $110.40 (Family) and $50.40 to $46.20 (individual). Excess Major Medical benefits include:
• A $50 per day in-hospital benefit
• $1,325 coinsurance reimbursement
• In-hospital Private Duty Nursing
• Outpatient Rehabilitation
• Up to 30 days care in a Nursing Home
• $15,000 AD & D benefit for employees
• Vision Care benefits every two years.
The Vision Care Plan is the most used benefit of the Excess Major Medical policy. Davis Optical is the preferred provider but you may elect to use any vision care provider to receive this benefit. Excess Major Medical forms, including vision forms, can be found on our website, HHHTA.com No vision
